Born June 21, 1967 in Idaho Falls, Idaho a daughter of Gary Rex and Bonnie Elaine Hall Ballard.
Survived by best friend and companion, Charles Tisdale; one daughter, Jessica Lee; two sons, Tory and Joshua, Salt Lake City; two sisters, Mrs. Bobby (Trina) Russom, Salt Lake City; Angelique Ballard, Carlin, Nevada; three brothers, Gary Trent Ballard, Troy Ballard, both of California; Alex Ballard, Carlin, Nevada; grandmother, Buelah Ballard, Idaho Falls.
Funeral services will be held Thursday, April 28, 1994, 1 p.m., at Larkin Mortuary, 260 East South Temple, where friends may call one hour prior to services. Interment will be Friday, April 29, 1994, 1 p.m. at Annis Little Butte Cemetery, Annis, Jefferson County, Idaho.
