According to the accident analysis report he was killed when the aircraft he was in developed engine trouble in the right propeller which caused it to feather in mid-air. The aircraft was believed by witnesses to have blown up or disintegrated in mid-air. No parachutes were deployed. The aircraft belonged to the 15th Army Air Force and the flight had originated in Italy. There were 6 crew members on board. They were Col (P) William Eaton, Major John C. Grobmeyer, 2Lt Alfred A. Faflik, Sgt Rocco R. Brangiforte, Sgt John W. Eschmann, Cpl V. Chicarelli. The local French FFI resistance forces recovered the bodies and placed them in the mayors building of the village of Chapel Montange. Chapel Montange lies 7 miles east of Vichy. BG Eaton and 2Lt Faflik are buried in the Rhone American Cemetery.
