Enlisted in U.S. Navy January 13, 1917, Great Lakes Naval Training Station, Chicago, Ill., under the name of Frank Moffett. Transferred to Philadelphia, Pa. Served on board the Minnesota, Kansas, Solace, and Ohio. Died August 27, 1917, on board Hospital Ship Solace during operation for acute appendicitis. Buried in Park Cemetery, (IOOF), Bourbon, Marshall County, Ind.
