According to her Death Certificate, Nellie was a widow when she died of nephritis at her home in Nashville, Tennessee, although the certificate didn't record the names of her parents or her deceased husband.

Nellie R Simmons Butler was the daughter of Sanford "Sandy" Simmons and Ann Davis. She lived with both parents in Panola County Mississippi in 1870. In 1875 she married George Frank Butler, a close neighbor in Panola County Mississippi. They lived in Sardis Mississippi a few houses over from her parents on the 1880 Census with their two oldest children Rev Sandy B Butler and Julia E Butler. In 1900, they lived in Sardis Mississippi with their children Leroy, Lloyd and Ruby. Sandy Butler has moved to Memphis and was a student at LeMoyne and later Western College in Springfield Missouri at this time. Julia most likely died at a young age. On the 1910 Census their last name is misspelled as Button and the ages are erroneous however the first names are correct. Lloyd and Ruby are both still living at home with them plus two other daughters that were born after 1900 named Lillie and Mallie who must have died before they relocated to Wichita Kansas sometime after 1912 when Sandy became Pastor
