Registered No. 3387, Dr. further certify that deceased was admitted to this institution on July 30, 1923, Dr. last saw him alive on the 31 day of July 1923, and he died on 31 day of July, about 6:10 PM, the diagnosis during his last illness was Polioencephalitis.
